What is sleep problems & sleep disorders?

Sleep problems can include difficulty falling asleep, repeated waking, early-morning waking, excessive daytime sleepiness, irregular sleep timing and sleep-disordered breathing. Not every sleep problem is the same, and treatment depends on the underlying pattern.

Common patterns and symptoms

  • Difficulty falling asleep or staying asleep
  • Waking earlier than intended or not feeling refreshed
  • Marked daytime sleepiness or fatigue
  • Irregular or delayed sleep timing
  • Snoring, witnessed pauses in breathing or gasping during sleep
  • Sleep problems that affect concentration, mood, work or safety

Understanding the pattern

Assessment includes sleep timing, duration, awakenings, daytime symptoms, medicines, caffeine/alcohol use, mental health, work schedules and relevant physical symptoms. A sleep diary can be useful.

Treatment may involve behavioural strategies, structured psychological treatment for insomnia, treatment of an underlying psychiatric or medical condition, or referral for specific sleep evaluation when indicated.

Persistent sleepiness that affects driving or safety deserves prompt attention. Loud snoring with witnessed breathing pauses, gasping or marked daytime sleepiness may need assessment for a sleep-related breathing disorder.

Assessment

A clinical assessment usually explores onset, duration, severity, functional impact, relevant medical history, current medicines or substances, sleep, mental health, relationships and any safety concerns. Tests or specialist referrals are guided by the history rather than ordered automatically.

What can treatment involve?

Treatment is individualised. Depending on the condition and its severity, care may include psychological therapy, behavioural strategies, medication, relationship or family work, lifestyle measures, or referral to another medical specialist when needed. The aim is to reduce distress and improve functioning rather than simply suppress a single symptom.

When should you seek help?

Consider a consultation when symptoms are persistent, distressing, worsening, interfering with work or study, affecting relationships, or leading to avoidance or repeated attempts to cope alone. Earlier assessment can sometimes prevent a problem from becoming more entrenched.

Urgent situations

If there is immediate danger, severe confusion, inability to care for basic needs, severe withdrawal, active suicidal intent, or another acute medical or psychiatric emergency, seek urgent care from the nearest emergency medical service or hospital.

Can this problem improve with treatment?

Many people improve when the underlying drivers are identified and treatment is matched to the pattern and severity of symptoms.

Do I need an assessment if symptoms are only occasional?

Occasional symptoms may be common, but assessment is sensible when the problem is persistent, distressing, worsening or affecting daily life or relationships.

Will treatment always involve medication?

No. Treatment depends on the condition, severity, contributing factors, preferences and medical context. Psychological, behavioural and medical approaches may be used alone or together.

How much sleep should an adult generally get?

Sleep needs vary, but adults generally need around 7–9 hours of sleep per night. More important than a single number is whether sleep is adequate, regular and restorative for the individual.
