How to recognize depression: when sadness becomes more than sadness

Everyone feels sad at times. Depression is different because a broader pattern of symptoms can persist, affect everyday functioning, and reduce interest or pleasure in life.

When does sadness become a clinical concern?

Sadness after disappointment, loss or stress is part of ordinary life. A clinical assessment becomes useful when low mood or loss of interest persists, is accompanied by other symptoms, or starts interfering with work, study, relationships or self-care.

You do not have to wait until symptoms become severe to ask for help. Persistent changes in sleep, appetite, energy, concentration, motivation or self-worth deserve attention.

How a psychiatrist assesses depression

Assessment looks at the full pattern, not one questionnaire score. A clinician may ask about onset, duration, functioning, previous episodes, family history, medicines, alcohol or other substances, physical health and major stressors.

Screening tools can help estimate symptom severity, but they do not replace a clinical assessment.

What treatment can involve

Depending on the severity and circumstances, treatment may include evidence-based psychological therapy, medication, lifestyle and behavioural measures, or a combination.

Treatment is adjusted over time according to response, side effects, preferences and relapse risk.

When urgent help is needed

Seek urgent help if there are thoughts or plans of suicide or self-harm, inability to remain safe, severe inability to care for basic needs, or rapidly worsening confusion or agitation.
